Patient Participation Group

We have a patient group which tries to meet up on a quarterly basis. We discuss changes within the practice and how we can improve patient experience. Our next meeting is on Tuesday 24th May at 6pm. New members are always welcome. If you would like to attend please let a member of the reception team know or alternatively email hortonbank.patients@nhs.net

We now have a dedicated telephone number for our elderly and housebound patients to ring to order their prescriptions. The number is 01274 503642 and is available Mon -Fri 11am -2pm. This number is also available for all patients to ring for their test results.

PHYSIO 1st. From 3rd May 2016 we will be introducing Physio 1st, a new self referral physiotherapy service. This is for new onset muscular, joint and back problems. Please contact the surgery for more information.

SMS text service is available to all mobile phone users to remind them of their appointments. Please ask at reception for a consent slip or you can register online. Please be aware it is the patients responsibility to inform the practice of any changes to mobile numbers. 

If you are unable to attend an appointment you can text your name and appointment details to 07754 805009 and a member of the reception team will cancel your appointment.

NAMED GP

You may be aware from April 2016 all surgeries are required to provide their patients with a named GP who will have overall responsibility for the care and support that the surgery provides. This is intended to continue the patient care you already recieve from the practice.

Important: This does not mean that you must always see this doctor. You are free to choose to see any of the GPs at the surgery. Also your named GP may not always be available.You may need to see a different doctor if you are acutely ill. If this is the case, your continuity of care will not be affected

Appointments

Our appointments become available at 8am and 2pm Monday to Friday. You can also pre-book an appointment upto 1 month in advance.

You can see any doctor, not just the doctor with whom you are registered. All surgeries are run by appointment although the practice is looking at ways of providing services that may meet demand in better ways.

If you are Pregnant and unable to get an appointment you can contact the ante-natal day unit at your local hospital who will be able to offer you advise.

If Your Case Is Not Urgent you will be offered the first available appointment with the doctor of your choice.

Appointment Cancellations

If You Cannot Keep Your Appointment please let us know so that it can be given to someone else. You can register for on line services where you can make and cancel appointments. Alternatively see the Text 2 Cancel information below. Please make every effort to be on time for your appointment. The practice loses approx. 60 appointments a week due to people defaulting their appointments.  If a patient fails to attend three appointments, without cancellation, in twelve months, their name will be removed from the practice's patient list.You have the right to appeal against this procedure by putting an appeal in writing to the Practice Manager and your case will be investigated further.

Nurse appointments are available as appropriate.
